This program provides case management, information & referral, state benefit application assistance, and outreach for the Greater Torrington and Winsted areas. The goals of the project are to provide outreach and engagement activities to address the social determinants of health for vulnerable populations through the development of action plans and linkages to employment, education, training, early childhood, housing, behavioral health, and basic needs assistance services.
